Will this make your back straight?
It can overtime. It forces you to keep your posture upright instead of having a lazy slouch. It is uncomfortable, but not to the point where you want to punch someone in the face. If you want to have good posture, then do it.
Are you supposed to wear this on your bare-back? Can you wear a shirt underneath?
It's your choice to make. I wear it with a shirt underneath but works both ways
Can you work out at the gym in this?
You could, but it won't be a comfortable experience. I only wear it while i'm at work for 4 hours, take it off, and then put it back on in the evening for an hour or two. It has changed the way I stand even when its off. You learn to stand correctly.

No. It’s for holding back your shoulders for people who slouch.
Wear while sleeping?
I would not recommend it while sleeping. You’re not supposed to wear for a long length of time, I only wear for about 4 hours a day while I’m working.
Are those straps that go around the mid torso comfortable? Do you wear it over your skin?
I don't find them comfortable. I wear the brace over a T-shirt.
